New Patient NHS Registration

We are currently welcoming new NHS patients at both our Alloa and Alva practices. Complete the short form below and our reception team will be in touch to arrange your initial examination appointment.

Annual NHS examinations are free of charge. You may be entitled to free treatment if you are under 26, pregnant or a nursing mother, or in receipt of certain benefits.

Please bring a list of any current medications and, if you are exempt from NHS charges, the relevant documentation to your first appointment. Your registration is activated on completion of your initial examination.
